If you are a national of a country that needs a visa to enter Italy, then you must submit an Italian visa application to the adequate Italian representation.
Anyone who needs to submit an Italy visa application must have a list of documents to support their application. The requirements may vary depending on the purpose and length of travel. However one document everyone must submit is an Italy Visa Application Form.
There are two types of Italian Visa Application Forms: for short-stays (Schengen Visa) and long-stays (National Visa).
This article is aimed to inform you how to fill an Italy Visa Application form.

Through this website, you can simply write all the required information on the adequate columns and click “Print”. This will provide you with a printable version of the visa application form with all of your information filled out.
You must then print it, sign it, and attach it to the rest of your documents when you submit your Italy visa application.

Keep in mind that different countries may have different requirements when it comes to completing the Italian Visa application form. Instead of filling it out online, you may be asked to download the application first and fill it in by hand, or you will be given the application form when you make the visa appointment.
You should always ask the Italian representation or Visa Application Center through which you are applying for more information.
Filling your Schengen Visa application form correctly is one of the most crucial parts of your Italy visa application. It is very difficult to obtain a Schengen visa for some countries, due to the high number of applications, so visa officials are known for sometimes rejecting applications for the smallest error – such as a mistake in the application form.
As such, when you are filling the application form, make sure to read every question carefully and answer it with the utmost care and precision.
Be specific. For example, when asked for an address, do not simply write the city and country. Write the specific, detailed address.
Make sure not to leave any fields unfilled. If one particular field does not apply to you, write N/A (Not Applicable).
Base your answers to the information on your passport.
The majority of questions on both the Italy Schengen Visa Application Form and the Italy National Visa Application Form are the same. We have specified any differences.
Write the last name that is written on your passport. If you use hyphens on your name, add them. If your current last name is your married last name, do not specify by adding “married name” or adding your husband’s name as well.
Simply write your last name as it is.
If you are married and have changed your last name, write your maiden name here (the last name you were assigned at birth).
Even if your current last name (in Question 1) and your birth last name are the same, write it.
Write your legal first name(s) here, the same as it’s written on your passport.
Make sure to use the day-month-year (DD-MM-YYYY) format when writing your date of birth! If your birthday is 1 March, 1980, for example, write 01-03-1980.
Write the place you were born here, in the same way as it’s written on your passport. Make sure the spelling of the place of birth is the same on both!
Write the country you were born in, regardless of whether you are currently using its passport or not.
Write your current nationality here.
If you had a different nationality at birth, specify it on the required field.
Tick the appropriate box. Provide your sex as indicated on your passport and/or ID.
Tick the appropriate box corresponding to your legal marital status.
If the applicant is a minor, you must specify their surname, first name, address (in case it is different from yours) and nationality of the parent/legal guardian.
If you have a national identity number (for example, the number on your ID), write it here.
Tick the type of travel document that you have.
Write the number that is written on your travel document.
Write the date that your travel document was issued as it is written on the document.
Write the date up to which the document is valid for, as it is written on the document.
Specify the authority which issued your travel document, as well as the place of issue.
Write your personal home and email address and any phone numbers you have (mobile and landline).
If you are using a mailbox that is owned by someone else, for example your company, specify the name of the owner.
For minors, specify whether this is the address/phone details of the parent or guardian.
If you are currently living in a country that’s different from your country of nationality, specify by ticking the adequate answer.
If you are living in another country with a residence permit, give information on what type of residence permit it is, the number of your residence permit, and its validity.
If you are a citizen, provide the citizenship number.
You should normally write the occupation that is indicated on your passport. However, if you have changed professions, you must indicate the change.
For example, if you were a student but now you work as a teacher, write “Teacher (former student)”.
If you are not employed, write “No Occupation”.
Write the name, address, telephone number, and email address of your place of work or educational institution.
Tick the most appropriate answer in regards to your purpose of travel.
For a Schengen Visa, you have to specify what Schengen member state you will stay in the longest. For an Italy Schengen Visa (if you will stay the longest in Italy), write “Italy”.
For an Italy National Visa, write the city in which you intend to stay in.
Write the Schengen member state through which you will be entering the Schengen Area/the first state you will visit.
If you won’t be staying in another state other than Italy, just state “Italy”.
Tick the appropriate answer.
Specify the number of days you intend to stay in Italy.
For an Italy Schengen Visa, you can stay up to 90 days.
For an Italy National Visa, the duration of intended stay can be a maximum of 365 days.
Specify whether you have been granted a Schengen visa within the past three years.
If yes, specify the dates during which it was valid.
Specify if you have given your fingerprints (biometrics) before for a Schengen visa.
If yes, write the date when you have given them.
Entry permit for the final country of destination. If your final destination is a country outside the Schengen area, specify the type and validity of the visa for that country here.
If required: The number of authorization for the national visa issued by the appropriate Italian Immigration Office (SUI). Specify the dates of validity as well.
This is the date in which you first intend to enter the Schengen zone (not necessarily in Italy, just the first Schengen country you will stop in).
The date in which you intend to leave the Schengen Area (not necessarily Italy, it’s just the last Schengen country you will visit).
If you do not have a sponsor, write the name of the other type of temporary accommodation, such as a hotel/hostel/rental home etc.
For Italy National Visa: The name of your family member in Italy (if applying for family visa), or employer (work visa). In case of other types of visa (such as Adoption, Religious reasons, Medical treatment, Sport, Study, Mission), write the Italian address where you are staying.
If an organization or company is sponsoring your trip, write the name, address, phone number, and the telefax of the organization.
You have two boxes specifying whether you will cover your own expenses to Italy or a sponsor will. You must also specify the method through which your expenses will be covered. You have a number of options to choose from. Tick the one that is most appropriate.
If you have a close family member who is an EU, EEA or CH citizen, write their personal information in the required fields.
Specify your relationship to your EU, EEA or CH citizen family member. For example, whether you are a spouse, child, or other dependent descendant to them.
Write the place and date in which you are currently filling out the Italy Visa Application Form even if it is different from the place and date when you attend your visa interview.
Once you print your Italy Visa application form, sign it.
If the applicant is a minor, a parent or legal guardian must sign it for them. Indicate the relationship to the minor in parenthesis next to the signature (mother, father, etc.)
