DuPage County Vital Records
The DuPage County Clerk's Office issues certified copies of DuPage County birth certificates, DuPage County death certificates, and DuPage County marriage certificates.
You may order copies of DuPage County vital records securely through VitalChek. Orders placed on VitalChek can also be processed on an expedited basis for an additional shipping fee.
NOTE: There is a $10.00 service fee to order certificates/records through VitalChek. The fee is not charged nor collected by the DuPage County Clerk's office. Expedited shipping fees are collected through UPS.
Our office holds birth certificates of individuals born in DuPage County, Illinois. For birth certificates of individuals not born in DuPage County, please contact either the county of birth or the Illinois Department of Public Health Division of Vital Records at (217) 782-6553 or (312) 814-2793. Please note: By law, the County Clerk's Office CANNOT make corrections to a birth record. Please contact the Illinois Department of Public Health - Vital Records Department for corrections.
Copies of Birth Certificates:
Who May Obtain a Birth Certificate?
Under Illinois State Law (410 ILCS 535-Vital Records Act) only specific individuals have legal access to birth, death, or marriage certificates. The DuPage County Clerk's Office will issue certificates to authorized individuals only. To do otherwise is a violation of Illinois Law. Vital records are not considered public information and are not subject to the Freedom of Information Act.
All requests, whether they are made in person, by mail, or online, must be accompanied by the Current and Valid Identification* of the requestor, or the request will not be fulfilled. Please see below for a list of examples of current and valid identification.
Fee Waiver for Homeless Individuals and/or Other Qualifying Individuals:
By law, homeless individuals (and other individuals in qualifying circumstances) in born in DuPage County, Illinois are entitled to request a free certified copy of their birth record. The individual’s homeless status must be verified by a human services agency, legal services agency, or another similar agency. Case Workers working with victims of domestic abuse/violence, DCFS representatives, and other agencies can apply for individuals who qualify for no-charge birth records.

What are the Fees for a Birth Certificate?
The fee is $14 for the first copy and $2 for each additional copy of the same certificate, requested at the same time.
What are the Acceptable Forms of Current and Valid Identification?

What About Genealogical Research?
Birth records had to occur 75 years ago or earlier to be classified as genealogy records. The application process is the same as above, however, the fee is $1 per record. Genealogical records are not certified, but stamped "For Genealogical Purposes Only."

What are the Fees for a Civil Union Certificates?
The fee is $14 for the first copy and $2 for each additional copy of the same certificate, requested at the same time.
Copies of Death Certificates:
DuPage County can provide certified death records for deceased individuals if the death occurred in DuPage County.
Death records for deaths occurring anywhere in Illinois can be requested through the Illinois Department of Public Health, Division of Vital Records.
Who May Obtain a Death Certificate?
You can obtain a certified copy of a death certificate if you are a direct relative or an individual who can prove a financial interest. Examples include:
